Dying Matters Awareness Week is a national initiative to encourage people to start talking about death, dying, and bereavement. This year’s theme focuses on the importance of being in a good place to die.

There is no right or wrong place to die; it will be different for everyone. But it is important for people to think about it, to talk about it and to plan for it.

With this in mind, we have created a helpful form for you to use when planning ahead and outlining your wishes before and after you die. It can be hard to talk about death and dying with those closest to us, so we hope this document is a good starting point for you.
