About our Living Well service

Our Living Well and Supportive Care Service supports those living with a life-limiting illness to improve their wellbeing and manage their symptoms more effectively. The programme we offer empowers people to live well, by equipping them with the skills and knowledge they need to achieve their goals and to live life with purpose.

We understand that people have different needs, and wellbeing means different things to different people; we therefore offer a range of support and activities, so that everyone can find something right for them. Our specialist team are by your side all the way, and the care and support provided is within a compassionate, inclusive, and supportive environment.

What to expect

When you first access the service, we will work alongside you to identify which elements of our programme you feel will be most beneficial, and together create a personalised eight-week programme.

You may wish to:

  • Meet other people in a similar situation.
  • Access support from our specialist team.
  • Learn skills to maintain your independence.
  • Build confidence towards achieving a particular goal.
  • Increase your knowledge and understanding of your illness and how to manage your symptoms.
